For clients utilizing health insurance, clients are responsible for all deductibles and copayments determined by your insurance plan. Payment plans can be discussed if needed.
Self-pay
If your insurance is not listed above, or you do not wish to use your insurance, you have the option of paying out of pocket. The self-pay rates are:
30 minute appointments: $75
45 minute appointments: $112
60 minute appointments: $150
An invoice can be provided upon request to submit to your insurance company for possible reimbursement. Please verify with your insurance that they will accept this. Payment plans can be discussed if needed.
Cancelation policy: a 24 hour notice is needed if you are unable to keep your appointment. Clients can be charged a no show or late cancelation fee of $75 for any appointments missed without a 24 hour notice.
